What goes into an FBA and a BIP

A functional behavior assessment (FBA) finds out why a behavior happens: it gathers records, interviews and direct observation, defines the behavior, and ends with a hypothesis about its function, such as attention, escape, access to items or automatic reinforcement. A behavior intervention plan (BIP) turns that hypothesis into a plan: replacement behaviors to teach, antecedent changes, reinforcement and consequence strategies, a crisis plan and how progress will be measured.

Our FBA template walks through each part in order: learner information and team, reason for referral, record review, indirect assessment, direct observation (ABC, scatterplot, setting), operational definitions, baseline, setting events and antecedents, hypothesis statements, the competing behavior pathway, an optional functional analysis summary, and recommendations. The BIP picks up from there and ends with review dates, criteria for success and team consent.

Every section has grey guidance text telling you what to write, so a newer BCBA or a school team gets a consistent, defensible document. Rating scales are left as blanks for the tool you use; we do not reproduce copyrighted assessment items.

FBA and BIP template questions

What is the difference between an FBA and a BIP?
The FBA is the assessment that explains why a behavior occurs. The BIP is the plan that uses that explanation to reduce the behavior and teach replacement skills.
Can I edit the templates?
Yes. They are Word documents that also open in Google Docs, with real headings and tables you can add to or remove.
Does the kit work for schools?
Yes. The structure fits school-based FBAs and BIPs as well as clinic and home programs. Check your district or state requirements for any sections they mandate.
Is there an FBA example?
Each section includes guidance on what to write and how, and the hypothesis statements follow the When, will, in order to format.
Do you include assessment tools like the FAST or QABF?
No. Those are copyrighted tools. The template has a place to record which indirect tool you used and its result.
